Student/Family Information Opt-In/Opt-Out Request

In accordance with FERPA (Family Educational Rights and Privacy Act), you have the right to opt out of including and displaying you and your student’s information in your school's ClassroomParent directory.

Please complete this form to specify your opt-out preferences:

IMPORTANT: If you take no action, we will assume that you want to opt in.

Sharing Level 1:

	You will receive PTO and school information sent through ClassroomParent
	Your student's name and homeroom shared.
	Your and other parent's names will be shared
	Your contact information (email and cell number) shared

Sharing Level 2:

	You will receive PTO and school information sent through ClassroomParent
	Your student's name and homeroom shared.
	Your and other parent's names will be shared
	Your contact information (email and cell number) is not shared

Sharing Level 3:

	You will receive PTO and school information sent through ClassroomParent
	Your student's name and homeroom shared.
	Your and other parent's names are NOT shared
	Your contact information (email and cell number) is NOT shared

Sharing Level 4:

	You will NOT receive PTO and school information sent through ClassroomParent
	Your student's name and homeroom shared.
	Your and other parent's names are NOT shared
	Your contact information (email and cell number) is NOT shared

Sharing Level 5:

	You will NOT receive PTO and school information sent through ClassroomParent
	Your student's name and homeroom are NOT shared.
	Your and other parent's names are NOT shared
