It depends a bit on the need, but some schools have used the Sign Ups/Volunteer Management feature for an event to collect permission.

Basically, you would do the following: 

1. Create an event (or use an existing event that describes the whatever it is you need permission for). In the description of the event, note that you will ask each parent to click the Sign Up button beside the permission request.
2. Add a "Permission Request" sign up. In the description include whatever language you need. Also add an instruction that they should type the name of the child they have giving permission for in the Note field when signing up.
3. Send a notification to the parents about the event. This will include the Sign up in the email.
